About CWC

The CONSIGLIO WELLNESS CENTER is a "convenient care" facility open to Blue Springs, Grain Valley, Hickman Mills, Lone Jack, and Smithville School District employees, retirees, and their dependents. The center is located on the top floor of the Paul Consiglio Education Center.

Medical staff include Physicians and Family and Pediatric Nurse Practitioners as well as various wellness providers focused on prevention and urgent care needs. While this is not a substitute for a primary care physician, it will provide a convenient and affordable option for our staff to receive basic and immediate health care.

Employees, dependents and district retirees are eligible to take advantage of these services with copayment based on their current insurance coverage.

Appointments will be taken by phone. Walk-ins are also welcome. Additional information will be provided at your employee convocation.

Patient Registration Forms

Important!

Please bring identification to the CWC to ensure eligibility!

Please send your Cigna card, employee identification or a copy of either with an unaccompanied spouse or dependent. Minor dependents (under 18) must be accompanied by a parent for their first visit to the office; they may then be unaccompanied upon obtaining phone consent for treatment. To speed up registration download and complete these forms. Then bring the forms with you or fax to 816-224-1364. Thank you!

"I just wanted to let you know what a fantastic experience I had at the Consiglio Wellness Center! They were so fast and friendly. When I called with a question about a less expensive prescription they actually called my pharmacy to see what options were available and then called me back without me even asking them to! I really appreciate the wonderful service I received at this clinic!"
- Kendra Carpenter (Grain Valley School District)

CWC Lab Capabilities

Typically, patient labs are “ordered” by their medical professional. In other words, a test is authorized by the nurse or doctor and the patient has that “order” fulfilled at a laboratory facility. The CWC is not such a laboratory or draw station, but we welcome our members if the test that is ordered can be more conveniently handled at the CWC AND it is among the following:

• Total Cholesterol
• High Density Lipoproteins
• Triglycerides
• Low Density Lipoproteins
• Non-High Density Lipoproteins
• TC/HDL ratio
• Glucose
• Hemoglobin A1c
• Mono test
• Pregnancy testing
• Strep test
• UA Dip/Automated

Sports Physicals

The CWC is pleased to announce a new service for District employees: Student Sports/Activities Physicals. If your son or daughter requires a sports physical, simply contact the CWC at 816-224-1740 to make an appointment. Drop-ins are always welcome but an appointment insures shorter wait times if the wellness center happens to be busy.

*Please bring your completed physical form. You can pick the form up at the CWC, or download it here:

Frequently Asked Questions (FAQ)

WHO IS ELIGIBLE FOR TREATMENT AT THE CWC?

Any employee of our member districts (Blue Springs, Grain Valley, Hickman Mills, Lone Jack, or Smithville), along with their spouses and dependents is welcome regardless of their insurance carrier.

AT WHAT AGE ARE DEPENDENTS SEEN?

Because we have a Pediatric Nurse Practitioner, your child may be as young as 6 months old.

CAN I JUST DROP INTO THE CWC?

Yes. Walk-ins are welcome but those with appointments will be seen before drop-ins. It always better to call ahead but we’re happy to see you regardless.

CAN I LEAVE A MESSAGE FOR AN APPOINTMENT IF THE CWC IS CLOSED?

Yes. We’ll call you back during the next office hours. Be sure to leave the best number to reach you.

CAN I GET AN APPOINTMENT QUICKLY?

Absolutely! We’ll work you in as soon as possible or work with your school schedule to make your visit as convenient as possible.

WHAT SHOULD I EXPECT WHEN I ARRIVE?

If it’s your first visit to the CWC, there will be some registration and profiling to be completed. But once you’re in the CWC system, your wait time will be minimal.

HOW MUCH DOES IT COST, AND DO I PAY WHILE I'M THERE?

Depending on your eligibility, there may be no charge but never more than $40 for your treatment. We accept cash, check or credit card. Your H.S.A. debit card is welcomed.

IF I NEED A PRESCRIPTION, HOW WILL THAT BE HANDLED?

You will fill your prescription at the pharmacy of your choice.

CAN I SPEED UP THE REGISTRATION PROCESS?

Yes, and we encourage you to do so. Click HERE for the documents you will be required to complete. Please download and complete the forms then either bring the completed forms with you or fax to: 816-224-1364.
